Clinical Application of TADs in Contemporary Orthodontics

Orthodontics has evolved at a rapid pace in recent years. One of the most disruptive advancements in the field was the introduction of temporary anchorage devices (TADs).The combination of TADs with modern orthodontic techniques, which are becoming ever more efficient, has significantly expanded the limits of our orthodontic treatments. This has allowed us to address increasingly complex malocclusions, which would have been unimaginable to treat without the aid of orthognathic surgery in most cases.In my presentation, I will showcase complex clinical cases, combining the use of TADs with different orthodontic systems, allowing me tocorrect severe malocclusions in the vertical, sagittal, and transverse planes.

Strategic Extractions: From 1 to 8

Extraction in orthodontics remains a relatively controversial approach. Although the trend in the orthodontic treatment has been, specifically in the last decades, toward the non-extraction option, it is not always possible to treat all malocclusions without taking out any teeth. While the premolars have been always considered the black sheep of the family, some factors, that include patient’s history, specific types of malocclusions, periodontal condition, status of teeth, oral hygiene and others, might modify the diagnosis, and accordingly affect the decision of which teeth to extract. Some extractions that were and still are considered a taboo for most of lay people and even dental professionals, are becoming more acceptable when properly justified. This might require specific biomechanical considerations and thoughtful application of therapeutic skills.
In essence, the objectives of this presentation are:
1- to identify some specific indications where teeth in different sectors of the arch are diagnosed to be removed;
2- to highlight the advantages and consequences of such unusual therapeutic decision;
3- to provide useful biomechanical considerations to be applied in every clinical situation.

Self-Ligating Braces: Truth or Myth?

Self-ligating braces have been used in orthodontics for over two decades. Their developers have claimed several advantages over traditional braces, including reduced friction, more efficient tooth movement, and a decreased need for extractions in many cases. Despite these claims, strong scientific evidence supporting the clear superiority of self-ligating systems remains limited. Nonetheless, many clinicians—including myself—continue to incorporate them into clinical practice based on experience and case-specific benefits. In this lecture, I will present insights drawn from my 15 years of clinical experience with self-ligating braces, focusing on:
• The history and evolution of various self-ligating systems
• A critical review of the scientific literature evaluating their effectiveness
• The proposed advantages and known disadvantages
• An evidence-based comparison with traditional bracket systems
• Clinical cases where self-ligating braces have shown particular efficacy

Conservative management of TMD cases in modern orthodontics

One-third of the adult population suffers from temporomandibular disorders (TMD) and it drastically affects the quality of life of the individual. No single treatment modality is efficient in reducing the pain but a multidisciplinary approach is required for the complete remission of the symptoms. Conservative management is recommended for most TMD patients. Occlusal splints, is considered one of the most effective methods of treatment along with orthodontic treatment. Many different clinical cases of TMJ discrepancies treated by different techniques will be presented. This presentation will discuss the management of TMD cases and interaction with orthodontic treatment.

Clinical Pearls in Deep Bite Management with Clear Aligners.

Deep-bite correction with aligners can be challenging, but with innovative protocols and strategic approaches, predictable outcomes are achievable. This lecture introduces a cutting-edge protocol designed to enhance treatment precision and efficiency in managing deep bites with Clear Aligners. Special emphasis will be placed on the integration of Temporary Anchorage Devices (TADs) to improve biomechanics and control vertical movements. Attendees will gain practical insights into leveraging aligner features, optimizing ClinCheck settings, and incorporating TADs to overcome the complexities of this malocclusion.
